How to Complete & Submit Fillable Forms
1. If you do not already have Adobe Reader, click this link to download the FREE PDF reader, Adobe Acrobat Reader: https://get.adobe.com/reader/
a. Click “Download Acrobat Reader”
b. Double click on the installer file, and follow on screen steps to finalize installation
2. View email from PFCA, and download the attached fillable PDF and double click the download file to open the document (will open in a browser tab).
3. Complete each section on the fillable form
4. Click the download arrow (located in the top right corner of the browser) to save the file (with your edits) to your DESKTOP.
5. Attached the completed forms and email back to [email protected].
